This test is done on the serum of patient.
Important facts
•The thyroid gland has large stores of harmones and has slow rate of normal turnover.
•Pituitary gland produces TSH which stimulates thyroid gland and leads to and distribution of stored harmones.
•TSH stimulates secretion of T4 and T3.
•TSH secretion is regulated by T4 and T3 (feed back inhibition ).
•TSH stimulation is also regulated by Thyrotropin-releasing harmone ( TRH ) from the hypothalanus.
•TSH is single most test for the primary hypothyroidism.
•If there is clear evidence of hypothyroidism and TSH is normal then think about the possibility of hypopituitarism.
•TSH level are high in primary hypothyroidism.
•TSH levels are low in the hyperthyroidism.
NORMAL
Adult : 0.4—4.2 mIU/L ( SI units are the same )
Neonates : 3-20 micrIU/L
INcreased level is seen in
1.adult and noenates with primary hypothyroidism.
2.Hashimoto’s thyroiditis.
3.Hypothroid patient rceiving inadequate treatment.
4.Thyrotoxicosis due to pituitary tumors.
5.Thyrotropin producing tumors e.g. ectopic TSH produced by the lung and breast cancers )
Decreased level seen in
1.Primary hyperthyroidism.
2.Secondary and tertiary hypothyroidism.
3.Over treatment in hypothyroid patient.
4.Treated Grave’s disease patient.
